Washington State Health Data Rights

If you are a Washington State resident, you have the right to access, delete, or withdraw consent for your consumer health data under the Washington My Health My Data Act. Use the form below to submit a request, or email us directly at privacy@zuralog.com.

We will respond to your request within 45 days as required by Washington State law. If we need additional time (up to 45 more days), we will notify you before the original deadline.

You can also submit your request by emailing privacy@zuralog.com directly with "Washington Health Data Request" in the subject line.

You can also manage your health data directly in the app without submitting a form:

• Delete your account and all data: Settings → Delete Account
• Withdraw consent for health data collection: Settings → Privacy → Revoke Health Data Consent
• Download a copy of your data: Settings → Privacy → Export My Data
